Job DescriptionAgawam West Rehab & Nursing is seeking an experienced LPN to join our team full-time as an MDS Coordinator.This salaried position is responsible for coordinating resident assessments, supporting care planning initiatives, and helping ensure accurate reimbursement and regulatory compliance. The ideal candidate is organized, detail-oriented, and knowledgeable in the MDS process within a skilled nursing environment.ResponsibilitiesCoordinate and complete MDS assessments and related documentationEnsure assessments are completed accurately and submitted timelyCollaborate with nursing, therapy, and interdisciplinary team membersParticipate in resident care planning and assessment reviewsSupport reimbursement and regulatory compliance through accurate documentationMaintain current knowledge of MDS and CMS guidelinesQualificationsActive Massachusetts LPN license requiredPrevious MDS experience in a skilled nursing facility preferredUnderstanding of MDS scheduling, coordination, and completionLong-term care or rehabilitation experience preferredStrong organizational and communication skillsCompensation & BenefitsSalary: $91,000 - $95,000 annually (DOE)Quality Measure Bonus: Up to $7,500 per yearSalary increases tied to censusMedical, dental, vision, 401(k), PTO, and moreThis is a direct hire position; we are not an agency.
