Hospice Services of Massachusetts is seeking per diem Hospice RN Case Managers to join our team of caring professionals. As a Hospice RN Case Manager, you will plan, organize, and direct hospice care as an RN, building from the resources of the community to meet the needs of individuals and families within their homes and communities.

$42 to $52 an hour, depending on experience, with weekly pay.

Responsibilities:

  • Supervise hospice aides

  • Complete comprehensive assessments of patients and families to determine hospice needs

  • Provide professional nursing care by utilizing all elements of the nursing process

  • Assess and evaluate patient's status by writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary

  • Initiate the plan of care and make necessary revisions as patient status and needs change

  • Develop a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions

  • Provide health care instructions to the patient as appropriate per assessment and plan

Experience & Education:

  • Graduate of an accredited school of nursing

  • One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care

  • Current licensure as an RN in Massachusetts and CPR certification

  • Bachelor’s degree, with one (1) year of hospice care experience preferred
