Job DescriptionOverview
Licensed Practical Nurse (LPN) – Long Term Care in Lebanon, New Hampshire. Start date August 5, 2026 for a 16-week travel assignment with a weekly pay range of $936 to $981 and guaranteed hours of 36 per week. Shift is 12-hour rotations (7:00 a.m. to 7:30 p.m.) on Wednesdays and Thursdays and every other weekend. Setting: LTC – Nursing Home.

Responsibilities
- Provide direct nursing care to residents in a long-term care setting, under the supervision of an RN or physician, including assessment and ongoing monitoring.
- Administer prescribed medications and treatments per care plans; perform routine procedures (e.g., wound care, injections where permitted) and monitor for adverse effects; document all nursing notes in the resident chart.
- Accurately collect and record vital signs; identify changes in condition and report promptly to the supervising nurse; participate in care plan updates and rounds.
- Assist residents with activities of daily living, mobility, hygiene, nutrition, and hydration; support safety and comfort to promote quality of life.
- Maintain infection control practices, proper use of PPE, and safe medication handling; ensure a clean, organized work environment and proper waste disposal.
- Communicate effectively with residents, families, and the interdisciplinary team; participate in care conferences and coordinate with therapy, social work, and other disciplines as needed.

Requirements
- Active LPN license in New Hampshire or eligibility to practice in NH; current standing as required by state guidelines.
- Current CPR/BLS certification.
- Ability to work 12-hour shifts as scheduled (7:00 a.m. to 7:30 p.m.) on Wednesdays and Thursdays and every other weekend; reliable attendance and punctuality.
- Experience in long-term care or geriatric nursing preferred.
- Strong communication, documentation, and teamwork skills; ability to work effectively within an interdisciplinary team.
- Eligibility for facility credentialing and regulatory requirements; able to complete necessary background checks and screenings as part of the assignment process.
