Job DescriptionRN/LPN Per Diem - Overnight Shifts
Our organization is in search of a Per Diem RN or LPN to deliver direct care to residents in a collaborative environment where the well-being of our residents is our top priority. Currently, we are focusing on filling Overnight shifts.

Responsibilities:
- Carry out professional responsibilities in line with facility policies, standards of practice, the New Hampshire Nurse Practice Act, OBRA, and HIPAA regulations.
- Work with interdisciplinary team members to develop personalized care plans for residents, updating them as needed, and participating in care plan meetings.
- Implement care protocols as required, including fall prevention, safety measures, behavior management, weight control, skin integrity, bowel and bladder management, infection control (Standard Precautions), and pain assessment and management.
- Evaluate and document any changes in residents’ physical or mental health, reporting findings to the primary care physician and treatment team, providing comprehensive clinical data for informed decision-making.
- Guide and oversee direct caregivers, preparing daily assignments and appropriately delegating tasks. Supervise direct care through observation and assessment of care outcomes.
- Foster positive relationships with team members, residents, and their families to encourage open and effective communication.
- Administer and document medications and treatments for residents in accordance with professional standards and nursing department policies, ensuring proper inventory management for narcotics.
- Encourage and maintain a positive culture for both team members and residents.
Qualifications:
- A minimum of a Nursing Degree or Diploma from an accredited institution is required.
- Experience in a long-term care facility and specialized knowledge in elderly care and rehabilitative nursing is preferred.
- Must possess a valid RN or LPN license in the State of New Hampshire. Certification in intravenous administration is preferred. Annual CEUs must be maintained, and CPR certification is required.
