\n ### Requirements
\n ✔ Active Vermont RN License
\n ✔ Minimum *3* Year of Home Health & Hospice RN Experience**
\n ✔ Current AHA BLS Certification
\n ✔ Home Health and/or Hospice experience preferred
\n ✔ Experience with **Homecare Homebase (HCHB)** EMR
\n ### Responsibilities
\n * Conduct patient admissions, assessments, and comprehensive evaluations.
\n * Develop and implement individualized care plans for home health and hospice patients.
\n * Provide skilled nursing care to patients of all ages in their homes.
\n * Collaborate with physicians, therapists, caregivers, and the interdisciplinary healthcare team.
\n * Educate patients and families on disease management, medications, and care plans.
\n * Monitor patient progress, document clinical findings, and adjust care plans as needed.
\n * Maintain accurate documentation using **Homecare Homebase (HCHB)**.
