Location: Falmouth, MA
Job Type: Contract
Duration: 13 Weeks
Shift & Schedule: Evening & Night Shifts | 3:00 PM – 11:30 PM or 11:00 PM – 7:30 AM | 8-Hour Shifts | Monday – Friday | 40 Hours/Week | Every Other Weekend & Holiday
Local Candidates Required: NA

Pay Rate: $1,900 – $2,032/week

Position Overview
We are seeking an experienced LPN to provide direct nursing care to residents in a skilled nursing and long-term care setting.
Required Qualifications

Current LPN license appropriate for the assignment.

Experience providing nursing care in a skilled nursing or long-term care setting.

Ability to administer medications and treatments and monitor resident conditions.

Strong clinical documentation, communication, and organizational skills.

Ability to supervise nursing assistants and coordinate resident care.
Responsibilities

Provide direct nursing care in accordance with established nursing standards, policies, and procedures.

Supervise and direct nursing assistants and coordinate shift activities.

Complete documentation for resident admissions, transfers, and discharges.

Review resident charts, medication orders, dietary requirements, treatments, and care plans.

Receive and document physician orders and update resident records and treatment plans.

Maintain detailed nursing notes documenting care and resident responses.

Communicate resident needs, admissions, room assignments, and changes in condition to nursing personnel.

Consult with physicians regarding treatment, rehabilitation, and changes in resident condition.

Prepare and administer medications and treatments according to physician orders and applicable requirements.

Perform nursing procedures, including catheterization, tube feedings, suctioning, dressing changes, colostomy care, and drainage bag care.

Collect clinical specimens and perform routine laboratory tests as required.

Monitor and document vital signs, including temperature, pulse, respiration, and blood pressure.

Educate residents and families about health and care instructions.

Maintain adequate medications, equipment, and nursing supplies.

Support dietary compliance and resident safety.

Participate in facility inspections and surveys as required.

Orient new residents and family members to the facility.
-
Maintain confidentiality of resident information and protect PHI.

Uphold resident rights and communicate effectively with residents, families, physicians, staff, and visitors.

Perform other nursing duties as assigned.
