Key Responsibilities\n

- Conduct comprehensive assessments and develop personalized treatment plans aimed at addressing the unique needs of each client.

- Work closely with interdisciplinary teams to create and implement positive behavior support plans that reflect best practices in mental health care.
- Complete documentation in compliance with regulatory requirements and organizational standards, ensuring timely communication of essential client information.
- Participate actively in licensing and accreditation processes to maintain quality care standards and improve service delivery.
- Forge collaborative relationships with community partners, state agencies, and other relevant stakeholders to enhance service coordination and resource allocation.
- Engage in continuous professional development activities, including attending workshops, networking events, and contributing to relevant committees to stay abreast of current best practices in geriatric mental health.
- Promote the mission and philosophy of Rebuilding Lives within the local community, advocating for the mental health needs of seniors.
- Perform additional duties as assigned by the Clinic Director that enhance client care and organizational effectiveness.
