Job DescriptionFoster Care Case Manager (Full-Time)

Pay: $25/hour (~$52,000/year)
Schedule: Monday–Friday

Position Overview

Seeking a full-time Foster Care Case Manager to join a multi-disciplinary treatment team providing intensive case management to youth. This role involves close collaboration with clients, families, foster homes, and community partners to support treatment planning, goal development, and overall client success. Participation in on-call services is required.

Key Responsibilities

  • Manage a caseload of youth, including individuals at high risk of psychiatric hospitalization or intervention

  • Build and maintain strong relationships with clients and collateral partners, including schools, therapists, agencies, funding partners, clinicians, supervisors, and foster homes

  • Set limits for youth and develop interventions and individualized goals in collaboration with guardians and providers

  • Initiate and monitor treatment planning and external recommendations

  • Ensure compliance with agency policies, procedures, and contract requirements

  • Complete timely and accurate documentation, reporting, and tracking in accordance with protocols

  • Participate in on-call coverage as needed

  • Attend meetings with internal teams and external partners

  • Represent the program on task forces and community boards when applicable

  • Maintain confidentiality and exercise sound judgment under supervisory guidance

  • Stay informed on trends and developments in human services and case management

  • Perform other related duties as assigned

Qualifications

  • Bachelor’s Degree required

  • LSW or LSWA license, or license-eligible (license must be obtained within 6 months of hire)

  • Ability to obtain and maintain required certifications, including CPR/First Aid

  • Strong interpersonal skills with the ability to build effective working relationships

  • Ability to read, analyze, and interpret client information, clinical documentation, and education plans

  • Ability to reason through practical problems and interpret instructions in various formats

  • Cultural competency across diverse populations

  • Bilingual or clinical competencies as applicable to the role

Additional Requirements

  • Valid driver’s license and reliable vehicle

  • Willingness to travel locally to foster homes, schools, and community sites

  • Ability to complete MAPP training and certification to train Spanish-speaking foster care providers in medication administration

  • Proof of adequate personal auto insurance, including required bodily injury and property damage coverage
