Travel Contract – Submission Requirements
Required
- 3+ years of current/recent experience — must have been worked within the last 6 months
- Must be willing to float to areas where the candidate has experience
- IV certified, or willing to obtain IV certification during onboarding
- All RTO (Requested Time Off) must be disclosed at the time of initial submission and approved in advance

- RTO cannot exceed 5 days
- Holiday RTO is not permitted
- COVID-19 vaccination required

- Declinations/exemptions not accepted
- Proof of vaccination must be uploaded with the profile
- Candidate must reside more than 50 miles from the facility

- Permanent address must be clearly listed in the profile
Highly Preferred
- Previous travel assignment experience
- Experience working in a rural or small facility
Important: Candidates who do not meet the required experience, vaccination, distance, IV certification/willingness, floating, and RTO requirements may not be considered.
