Physical Therapist Per Diem
We are seeking a licensed Physical Therapist (Per Diem) to join our growing healthcare team. This role is ideal for a self-motivated clinician looking for flexible, as-needed work while providing high-quality, patient-centered care.
Responsibilities:
- Perform patient evaluations and reassessments
- Develop and implement individualized treatment plans
- Provide therapeutic exercises, manual therapy, and patient education
- Monitor patient progress and modify care plans as needed
- Complete accurate and timely clinical documentation
- Communicate effectively with patients, families, and care team members
- Comply with all healthcare, safety, and ethical standards
Qualifications:
- Active Physical Therapist license
- Ability to work independently and manage time efficiently
- Strong communication and interpersonal skills
- Reliable transportation (for home health visits, if applicable)
- Home health experience preferred but not required
Position Details:

Pay (Per Visit):
- Physical Therapy Evaluation: $90 per evaluation
- Physical Therapy Visit: $65$75 per visit
