Job Title: RN – Home Health
Location: South Dennis, MA
Duration: 13 Weeks
Schedule
- Hours: 40 hours per week
- Shift: Monday–Friday
- Additional: Occasional weekends and holidays
- Alternative Schedule: 4x10-hour shifts may be considered
Compensation
- Travel Candidates: $70 – $74/hour ($2,800 – $2,960/week, based on experience)
- Local Candidates: $60 – $65/hour ($2,250 – $2,437/week, based on experience)
Position Overview
A healthcare organization is seeking an experienced Home Health Registered Nurse for a 13-week assignment. The RN will provide comprehensive in-home patient care, ensuring high-quality clinical outcomes through assessment, planning, implementation, and evaluation of individualized care plans.
Coverage Area Requirement
- Coverage area includes Brewster and Harwich
- Candidates must secure housing within the Cape Cod region (mid-Cape or closer).
Key Responsibilities
- Perform comprehensive patient assessments, including physical, psychosocial, environmental, and health history evaluations
- Develop and implement individualized care plans based on assessment findings
- Monitor and evaluate patient progress, revising care plans as needed
- Maintain documentation that accurately reflects patient care, teaching, and outcomes
- Coordinate care with interdisciplinary teams and participate in case conferences
- Supervise Home Health Aides and review care plans every 14 days
- Communicate significant patient condition changes to physicians and care team members
- Obtain and verify physician orders for all care provided
- Plan and coordinate patient discharges and complete required documentation
- Refer patients to community resources when appropriate
- Ensure compliance with clinical standards, benchmarks, and regulatory requirements
Qualifications
Required:
- Active Massachusetts RN License
- Minimum 1 year of nursing experience (or 6 months with BSN)
- CPR Certification
- Successful completion of pre-employment medication test
Preferred:
- Home Health or IV therapy experience
- BSN degree
- Strong computer and documentation skills
