Responsibilities:
- Assess, plan, and implement individualized care plans for adult day health participants
- Monitor and document patient health status, vital signs, and changes in condition
- Administer medications and treatments in accordance with physician orders
- Coordinate with interdisciplinary care teams, physicians, and family members
- Provide health education and wellness guidance to participants and caregivers
- Respond to medical emergencies and provide first-line clinical intervention
- Maintain accurate and up-to-date clinical documentation and compliance records
Requirements:
- Active Registered Nurse (RN) license in the state of Massachusetts
- Current BLS/CPR certification required
- Experience in geriatric, adult day health, or community-based care preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills with patients and families
- Ability to work collaboratively in a multidisciplinary team environment
- Compassionate, patient, and dedicated to serving the adult population
