Job DescriptionAbout the Role:Solara Hospice LLC is looking for a compassionate and dedicated Hospice CNA to join our caring team in Waltham, MA. Responsibilities:

  • Provide personal care services to hospice patients including bathing, grooming, dressing, and oral hygiene
  • Assist patients with mobility, transfers, and positioning to ensure comfort and safety
  • Monitor and report changes in patient condition to the supervising nurse or care team
  • Deliver compassionate end-of-life care in alignment with each patient's individualized care plan
  • Support patients and families with emotional comfort and a reassuring presence
  • Document patient care activities accurately and in a timely manner
  • Collaborate with interdisciplinary hospice team including nurses, social workers, and chaplains

Requirements:

  • Current and valid CNA certification in the state of Massachusetts
  • Prior hospice, palliative care, or home health experience preferred
  • Strong understanding of end-of-life care principles and patient dignity
  • Excellent communication and interpersonal skills with patients and families
  • Reliable transportation and ability to travel to patient homes/long term care facilities in the Waltham, MA area
  • Ability to work independently while remaining a collaborative team member
  • Compassionate, patient, and emotionally resilient demeanor
