• Conduct patient evaluations and assessments to identify strengths, weaknesses, and goals

• Implement and modify treatment plans as needed, including exercises, stretches, and modalities

• Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination

• Participate in patient education and discharge planning to promote optimal outcomes and independence
