Position: RN/LPN - Full Time (32 hours per week, 11 AM to 7 PM)
Sign-On Bonus: $3000**
We are currently in search of a Full-Time RN/LPN to deliver direct care to residents in a collaborative environment where our residents are the focal point of our mission. This position includes a weekend commitment, requiring work every other weekend.

Key Responsibilities
- Carry out professional duties in line with facility policies, standards of practice, the New Hampshire Nurse Practice Act, OBRA, and HIPAA regulations.
- Work collaboratively with interdisciplinary team members to develop personalized care plans for residents, updating them as needed and participating in care plan meetings.
- Implement care protocols as required, including fall prevention, safety measures, behavior management, weight management, skin integrity, bowel/bladder management, infection control, and pain assessment and management.
- Assess and document any changes in residents’ physical or mental health, reporting findings to the primary care physician and treatment team to provide comprehensive clinical data for decision-making.
- Lead and guide direct caregivers, preparing daily assignments and appropriately delegating tasks while supervising care through observation and evaluation.
- Foster positive relationships with team members, residents, and their families to encourage open communication.
- Administer and document medications and treatments for residents according to professional guidelines and nursing department policies, ensuring proper inventory management of narcotics.
- Promote and maintain a positive culture for both team members and residents.
Qualifications
- A minimum of a Nursing Degree or Diploma from an accredited institution is required.
- Experience in a long-term care facility and specialized knowledge in elderly care and rehabilitative nursing is preferred.
- A valid RN or LPN license in the State of New Hampshire is mandatory. Certification in intravenous administration is preferred. Annual CEUs must be maintained, and CPR certification is required.
