Responsibilities
- Conduct comprehensive speech-language assessments for adult residents and develop individualized treatment plans

- Perform bedside swallow evaluations and provide dysphagia management within scope of practice
- Document evaluations, goals, progress notes, and discharge plans accurately in patient records
- Collaborate with an interdisciplinary team to support patient care plans and transitions of care
- Educate patients, families, and facility staff on communication strategies and safe swallowing techniques

Requirements
- Ability to work in a skilled nursing facility setting and collaborate with an interdisciplinary team
- Strong assessment, treatment planning, and documentation skills for speech-language pathology
- Capable of delivering therapy within an acute-care focused SNF environment
