Position: Full-Time RN/LPN (32 hours/week; 3 PM - 11 PM)
Sign-On Bonus: $3000**

Key Responsibilities
- Carry out professional duties in line with facility policies, standards of practice, and relevant regulations including the State of New Hampshire Nurse Practice Act, OBRA, and HIPAA.
- Work collaboratively with interdisciplinary team members to develop personalized care plans for residents, updating them as needed and participating in care plan meetings.
- Implement care protocols as required, including fall prevention, safety measures, behavior management, weight management, skin integrity, bowel/bladder management, infection control, and pain assessment.
- Evaluate and document any changes in residents’ physical or mental health, reporting findings to the primary care physician and treatment team to support informed decision-making.
- Provide leadership and guidance to direct caregivers, preparing daily assignments and appropriately delegating tasks while supervising care through observation and evaluation.
- Foster positive relationships with team members, residents, and their families to encourage open communication.
- Administer and document medications and treatments in accordance with professional guidelines and nursing policies, ensuring proper inventory management of narcotics.
- Promote and maintain a positive culture for both team members and residents.
Qualifications
- A minimum of a Nursing Degree or Diploma from an accredited institution is required.
- Experience in long-term care and specialized knowledge in elderly and rehabilitative nursing is preferred.
- Must possess a valid RN or LPN license in the State of New Hampshire. Certification in intravenous administration is preferred. Continuing education units (CEUs) must be maintained annually, and CPR certification is required.
