Responsibilities:

Conduct thorough assessments of patients' medical, physical, and social needs, using the Minimum Data Set (MDS) to inform care plans

Develop and implement individualized care plans that address patients' unique needs and goals

Collaborate with interdisciplinary teams to ensure comprehensive care and address any concerns or issues that arise

Monitor patients' progress and adjust care plans as needed to ensure optimal outcomes

Maintain accurate and detailed records of patient assessments, care plans, and progress
