What We’re Looking For

NAVIX Diagnostix is seeking an experienced Remote Patient Monitoring Clinical Lead (RN) to build, lead, and scale our RPM and CCM service lines across multiple client sites.


In this role, the Clinical Lead provides clinical oversight, program development, and operational leadership for a turnkey RPM & CCM model, including device deployment, daily patient monitoring, escalation protocols, and CMS-compliant documentation and billing support. This position plays a key role in ensuring services are clinically sound, compliant, and scalable while supporting improved patient outcomes and practice transformation.


The ideal candidate brings strong clinical expertise, leadership experience, and the ability to design and implement scalable care management programs in a remote, multi-site environment.


Essential Job Responsibilities

  • Lead the development, implementation, and scaling of RPM & CCM programs across multiple client sites
  • Develop and maintain clinical protocols, policies, and workflows, including patient eligibility, care plans, escalation thresholds, and documentation standards
  • Ensure providers maintain appropriate clinical oversight and final decision-making authority within established escalation pathways
  • Provide oversight of all clinical monitoring activity, ensuring timely and appropriate intervention
  • Supervise and support RPM & CCM clinical staff, including LPNs, Medical Assistants, and monitoring personnel
  • Review patient alerts, trends, and escalations for clinical appropriateness and risk
  • Serve as the escalation point prior to provider involvement, ensuring appropriate clinical decision pathways
  • Oversee development and maintenance of comprehensive patient care plans
  • Ensure monthly non-face-to-face care management time requirements are met and properly documented
  • Lead RPM & CCM program implementation across new client sites (30–45 day go-live model)
  • Standardize workflows across client locations
  • Ensure integration of monitoring devices, platforms, and client EHR systems
  • Build scalable staffing models aligned with patient volume and program growth
  • Serve as the clinical liaison to physicians, advanced practice providers, and clinical staff
  • Provide education on workflows, escalation protocols, and quality outcomes
  • Monitor care plan adherence, patient compliance, and care gaps
  • Ensure documentation supports medical necessity, time-based billing, and audit readiness
  • Ensure clinical staff maintain accurate, audit-ready time tracking
  • Ensure adherence to all regulatory and compliance requirements
  • Establish, monitor, and report key performance indicators (KPIs) aligned with client goals
  • Support program expansion, optimization, and continuous improvement initiatives


Minimum Qualifications

Education & Licensure

  • Active Registered Nurse (RN) license
  • Compact license strongly preferred or ability to obtain multi-state licensure

Experience

  • Minimum of 5 years of clinical experience (cardiology, chronic care, or population health preferred)
  • Experience with care management, RPM, CCM, or telehealth programs
  • Experience with clinical documentation and regulatory compliance
  • Leadership experience managing clinical teams

Skills and Competencies

  • Strong understanding of value-based care models and practice transformation
  • Excellent clinical judgment and escalation management skills
  • Ability to design and scale clinical programs in a multi-site environment
  • Strong communication skills with physicians, administrators, and clinical teams
  • Process-driven mindset with strong focus on compliance and quality
  • Ability to translate clinical activity into measurable operational and financial outcomes

  • Frequent use of computer systems and communication platforms

  • Occasional travel for client implementation and program support
