Job Requirements
- Minimum 2 years of experience
- OASIS (Outcome and Assessment Information Set) experience – REQUIRED
- Home Health and Hospice experience – REQUIRED
- Experience traveling patient-to-patient for home visits
- Ability to manage and adjust to schedule changes
- Active state RN license
- Valid driver’s license in good standing
- BLS/BCLS certification required
- EMR Experience: HealthWyse (Careficient) preferred
- Facility conducts its own background check
