JOB DESCRIPTION

We are seeking a compassionate, organized, and client-focused Case Manager to support individuals in accessing essential services, achieving personal goals, and maintaining independence. In this role, you will work directly with clients, families, and community service providers to coordinate care, monitor progress, and ensure clients receive the resources and support they need. The ideal candidate has a background in healthcare, human services, or social work and is passionate about making a positive impact within the community.

Responsibilities

- Conduct client assessments to identify individual needs and service requirements.

- Develop, implement, and regularly review individualized service and support plans.

- Coordinate and monitor services to ensure clients receive appropriate care and resources.

- Maintain ongoing communication with clients, families, caregivers, and service providers.

- Track client progress and provide follow-up support as needed.

- Accurately maintain confidential client records and documentation.

- Prepare reports and complete required case management documentation in accordance with organizational and regulatory standards.

- Participate in case reviews, team meetings, and service planning discussions.

- Advocate for clients and connect them with appropriate community resources and support services. Additional responsibilities: - Conduct in-person client visits and assessments as assigned. - Attend team meetings, case review meetings, and care planning sessions.

- Coordinate services with clients, families, healthcare providers, and community organizations.

- Complete documentation and submit reports within established timelines.

REQUIRED SKILLS AND EXPERIENCE

-Bachelor's degree

- Background in healthcare, social services, human services, or a related field.

- Prior experience in case management, care coordination, social services, behavioral health, healthcare, or community-based programs.

- Strong verbal and written communication skills.

- Excellent organizational, interpersonal, and problem-solving abilities.

- Ability to work independently while collaborating effectively with a multidisciplinary team.

- Strong commitment to client confidentiality, ethical practices, and person-centered care.
