Job DescriptionPhysical Therapist (PT) – Full-Time
Berlin Health & Rehabilitation Center is seeking a dedicated Full-Time Physical Therapist (PT) to join our in-house rehabilitation team in Barre, Vermont.
Pay: $42–$52 per hour
We provide person-centered care focused on helping residents and patients improve function, maintain independence, and achieve meaningful rehabilitation goals. Our interdisciplinary team works collaboratively to provide high-quality, compassionate care.

Physical Therapist Responsibilities

Evaluate patients and residents to determine their physical therapy needs, functional limitations, and rehabilitation goals.

Develop individualized treatment plans based on evaluation findings and patient goals.

Provide skilled physical therapy interventions to improve mobility, strength, balance, coordination, function, and independence.

Administer medically prescribed treatments in accordance with the established plan of care.

Monitor patient progress and response to treatment and modify interventions as clinically appropriate.

Provide education and guidance to patients, families, and caregivers regarding treatment plans, exercises, mobility, and safety.

Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary team to coordinate patient care.

Document evaluations, treatment sessions, progress, and other required clinical information accurately and timely.

Maintain compliance with applicable federal and state regulations, professional standards, and facility policies.

Participate in required meetings and other duties as assigned by the Director of Rehabilitation.
Physical Therapist Qualifications

Active and valid Physical Therapy license in Vermont.

Previous experience in a skilled nursing facility (SNF) or long-term care setting preferred.

Current knowledge of physical therapy treatment practices, techniques, and interventions.

Familiarity with Net Health or similar electronic documentation systems preferred.

Strong clinical, communication, organizational, and interpersonal skills.

Ability to work effectively with patients and residents with a variety of needs and abilities.

Ability to collaborate effectively with an interdisciplinary healthcare team.

Commitment to providing compassionate, patient-centered care.

Must be in good standing with all applicable licensing boards and regulatory agencies.
