Job DescriptionPart-Time RN/LPN Position - 10 Hours Weekly
Schedule: 8:30 AM - 1:30 PM, Two Days a Week
We are currently in search of a dedicated RN or LPN to join our team, providing direct care to residents in a collaborative environment where their well-being is our top priority. This position will focus on supporting our Wellness Clinic within Independent Living, under the guidance of the Wellness nurse practitioner. The ideal candidate should be available to work two days each week.

Key Responsibilities:
- Execute professional duties in alignment with facility policies, standards of practice, the New Hampshire Nurse Practice Act, OBRA, and HIPAA regulations.
- Work collaboratively with interdisciplinary team members to deliver patient-centered care for independent residents, participating in team meetings.
- Evaluate and document any changes in residents’ physical or mental health, reporting findings to the primary care provider or Wellness Clinic nurse practitioner, and providing comprehensive clinical data for informed decision-making.
- Conduct regular visits with residents for health monitoring, wound care, and medication management assistance.
- During walk-in clinic hours, independently assess and treat minor illnesses and injuries within the nursing scope, escalating to higher care levels when necessary. Respond to urgent resident needs via the “R-call” alert system and initiate EMS support as appropriate.
- Provide leadership and guidance to direct caregivers, supervising care through observation and evaluation of outcomes.
- Administer and document medications and treatments for residents in accordance with professional guidelines and departmental policies, ensuring proper inventory management of narcotics.
- Foster positive relationships with team members, residents, and their families to encourage open communication.
- Promote and maintain a positive culture for both team members and residents.
Qualifications:
- A minimum of a Nursing Degree or Diploma from an accredited institution is required.
- Experience in a long-term care facility and specialized knowledge in elderly and rehabilitative nursing is preferred.
- A valid RN or LPN license in the State of New Hampshire is mandatory. Certification in intravenous administration is preferred. Annual CEUs must be maintained, and CPR certification is required.
