Job Summary
"Navitas Healthcare, LLC" is seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing services to patients within the Brewster and Harwich service areas. The RN will perform comprehensive patient assessments, develop individualized care plans, coordinate care services, and provide ongoing patient education while ensuring high-quality home health care delivery.
Important: Travelers must secure housing on Cape Cod (preferably Mid-Cape). Candidates residing off-Cape or in Falmouth will not be considered.
Key Responsibilities
- Perform comprehensive nursing assessments upon admission
- Develop, implement, and revise individualized patient care plans
- Evaluate clinical needs and coordinate interdisciplinary services
- Participate in patient case conferences and care planning meetings
- Monitor patient progress and treatment effectiveness
- Notify physicians and care providers of significant changes in patient condition
- Provide patient and caregiver education
- Supervise Home Health Aide care plans every 14 days
- Obtain and clarify physician orders for care services
- Complete admission, visit, and discharge documentation accurately and timely
- Coordinate patient discharges and document outcomes appropriately
- Refer patients to community resources when needed
- Maintain clear and accurate clinical documentation
Required Qualifications
- Active Massachusetts RN License
- Minimum one (1) year of nursing experience
OR
- Six (6) months of nursing experience with a BSN
- CPR Certification
- Successful completion of VNA pre-hire medication test
- Strong assessment, communication, and care coordination skills
