MDS Coordinator

Job Summary:

We are seeking a highly skilled and dedicated MDS Coordinator to join our team at Sweetwater Care in Boston, MA. As a MDS Coordinator, you will play a crucial role in ensuring the accuracy and compliance of our Minimum Data Set (MDS) assessments, while providing exceptional patient care and support to our residents and families. About the Role:

As a MDS Coordinator at Sweetwater Care, you will be responsible for:

  • Conducting MDS assessments, reviewing and updating as needed, to ensure accurate and comprehensive assessments of our residents

  • Collaborating with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of our residents

  • Providing education and support to residents, families, and caregivers to ensure a smooth transition and successful outcome

  • Maintaining accurate and up-to-date records and reports, including MDS assessments, care plans, and resident information

  • Participating in quality improvement initiatives to promote best practices and enhance resident outcomes

Responsibilities:

  • Conduct MDS assessments in accordance with federal and state regulations, and Sweetwater Care policies and procedures

  • Review and update MDS assessments as needed to ensure accuracy and completeness

  • Collaborate with interdisciplinary teams to develop and implement individualized care plans

  • Provide education and support to residents, families, and caregivers to ensure a smooth transition and successful outcome

  • Maintain accurate and up-to-date records and reports, including MDS assessments, care plans, and resident information

  • Participate in quality improvement initiatives to promote best practices and enhance resident outcomes

  • Stay current with changes in MDS regulations, policies, and procedures

Requirements:

  • 2 years of experience in long-term care or related field

  • LPN or RN licensure required
