\n ### Position Overview
\n Maristhill Nursing and Rehabilitation Center is seeking an experienced **Physical Therapist (PT)** for a 13-week contract assignment in a **Skilled Nursing Facility/Long-Term Care** setting.
\n **Skilled Nursing experience is required.**
\n The Physical Therapist will work with residents, caregivers, and interdisciplinary healthcare teams to provide individualized physical therapy services focused on improving functional abilities, mobility, independence, and quality of life.
\n ### Key Responsibilities
\n * Evaluate residents and develop individualized physical therapy plans of care.
\n * Establish short- and long-term functional goals based on resident needs.
\n * Provide therapeutic interventions and monitor residents' progress toward established goals.
\n * Modify treatment plans as needed based on resident response and functional progress.
\n * Collaborate with physicians, nursing staff, caregivers, and other members of the healthcare team.
\n * Participate in interdisciplinary team meetings and assist with discharge planning.
\n * Provide resident and caregiver education regarding treatment plans, exercises, mobility, and assistive devices.
\n * Recommend and obtain appropriate assistive devices and educate residents on their safe use.
\n * Appropriately refer residents to Physical Therapist Assistants (PTAs) and collaborate with therapy staff.
\n * Maintain accurate and timely documentation in the Electronic Medical Record.
\n * Ensure documentation meets Medicare, Medicaid, private-pay, and other reimbursement requirements.
\n * Maintain compliance with applicable regulatory, professional, and facility standards.
\n * Provide clinical education and in-service training to nursing and other departments when appropriate.
\n * Supervise assistant-level staff as applicable and ensure compliance with state and professional standards.
\n * Maintain current clinical knowledge, licenses, certifications, and competencies.
\n * Follow infection control procedures and maintain a clean and safe clinical environment.
\n ### Required Qualifications
\n * **Active Massachusetts Physical Therapist License – Required**
\n * **Bachelor’s Degree – Required**
\n * **Minimum 3 years of experience**
\n * **Skilled Nursing Facility (SNF) experience – Required**
\n * Long-term care experience preferred
\n * **BLS/BCLS – Preferred**
\n * Strong clinical assessment and decision-making skills
\n * Excellent written and verbal communication skills
\n * Strong documentation and time-management abilities
\n * Ability to work effectively within an interdisciplinary team
\n * Proficiency with Microsoft Office
\n * Ability to read, write, speak, and understand English
\n ### Preferred Qualifications
\n * Master’s or Doctorate in Physical Therapy
\n * Previous long-term care/SNF experience
\n * Experience working with geriatric residents
\n * Experience with Medicare/Medicaid documentation and reimbursement requirements
\n * Experience supervising or collaborating with PTAs
\n ### Physical Requirements
\n * Ability to sit, stand, walk, bend, stoop, twist, and reach as required.
\n * Ability to lift up to **50 lbs.**
\n * Loads above 50 lbs. require a two-person lift.
\n * Frequent lifting/carrying of up to 20 lbs.
\n * Ability to perform essential job functions with or without reasonable accommodation.
