Home Health Registered Nurse (RN)
 Hillsborough County, NH | Flexible Schedule | Field Visits
Alfa Care LLC is a growing, locally owned home health agency in New Hampshire seeking an experienced Home Health RN to provide skilled nursing care to patients in their homes.
This role is ideal for an RN who values autonomy, meaningful patient relationships, and high-quality one-on-one care.
 
Key Responsibilities
<ul>
<li>Perform skilled nursing visits (SOC, routine, recertification, discharge)</li>
<li>Complete assessments and documentation (OASIS / Skilled Nursing Notes)</li>
<li>Develop, implement, and update individualized Plans of Care (POC)</li>
<li>Communicate patient status changes with physicians and care team</li>
<li>Supervise and support LNAs per state and federal guidelines</li>
<li>Educate patients and families on care, medications, and safety</li>
<li>Participate in case conferences and quality improvement activities</li>
</ul>
 
Qualifications
<ul>
<li>Active NH RN license (in good standing)</li>
<li>Minimum 1 year RN experience (home health preferred)</li>
<li>Current CPR certification</li>
<li>Ability to pass criminal background check</li>
<li>Reliable transportation for local field visits</li>
<li>Strong clinical judgment and documentation skills</li>
