Home Care Team Manager

Position Summary: This position is responsible for ensuring the delivery of quality case management to elders in the community through hiring, training, developing, supervising, and coaching case managers.

Principal Accountabilities:

Ensures that case managers receive supervision and direction:

  • Provides supervisory oversight of case managers by maintaining an open door policy and conducting regular supervisory meetings to monitor and evaluate performance.
  • Identifies training needs and coordinates training.
  • Conducts team meetings; coordinates team activities.
  • Oversees and coordinates specialized programs including Enhanced Community Options Program Independence Plus, Veterans Independence Plus, and Consumer Directed Care.

Ensures high quality case management:

  • Completes record reviews and runs quality assurance reports to ensure that phone calls, visits, and other activity is completed in accordance with Elder Affairs and agency requirements and within the mandated timeframe.
  • Meets regularly with case managers to review cases, service plans, and program enrollment.
  • Accompanies case managers on visits to monitor case management quality.
  • Collaborates with other programs and agencies.
  • Collaborates with the contracts administrator regarding vendor issues.
  • Acts as an advocate for client choice and self-direction.

Ensures that case managers are trained and developed:

  • Orients new case managers into a professional work environment; mentors experienced case managers to assist them in achieving their professional goals.
  • Promotes teamwork and team building and fosters positive morale.
  • Provides feedback and support to case managers in routine and crisis situations.
  • Fosters independence and professional growth.
  • Monitors adherence to agency and Elder Affairs rules and regulations; implements disciplinary action when necessary.

Ensures that service plans conform to the current spending plan for each home care program:

  • Monitors service level costs.
  • Resolves billing issues and vendor discrepancies with the Finance and Quality units.
  • Serves as an active participant of the supervisors team:
  • Fosters an interdisciplinary approach.
  • Shares in rotating coverage responsibilities such as middle manager of the day and triage.
  • Attends agency supervisors’ meetings.
  • Participates in policy discussions and procedure development.

Qualifications:

Education: Bachelor’s degree in human services or related discipline.

Experience: Minimum of two years elder services or other human services experience.

Special Skills/Knowledge: Excellent oral and written communications skills, ability to effectively relate to a wide variety of personalities, basic computer skills, organizational and time management skills, ability to multi-task and work in a fast-paced, high volume work environment, demonstrated leadership ability. Driver’s license and reliable transportation required.
