With an amazing opportunity to join a local franchise owned and operated locally homecare group AND while also partnering with a locally owned mobile out patient private practice. Boost Home Health provides holistic and clinician inspired services to patients in their homes. Maximize Your Mobility is an outpatient mobile practice providing PT and OT services to patients at home. Together we provide a continuum of care not available with traditional practices.

A Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. Treatments and services assisting patients with conditions including orthopedics, neurology, medical management and more. The ideal candidate is a patient, reliable and detail-oriented professional who encourages and inspires their patients to work toward greater independence and recovery.

Services performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Supervisor or Manager.

Compensation is pay per visit. Follow-up visits differ from admission and evaluation visits.

What you’ll be doing:

  • Provide physician prescribed physical therapy.
  • Improve or minimize residual physical disabilities of the patient.
  • Return the individual to optimum and productive level within the patient’s capability.
  • Participate with interdisciplinary team ensuring high level of patient care planning.
  • Direct and supervise personnel as required.
  • Responsible for initial assessment, plan of care, maintenance program development and modifications and reassessments every 30 days.

  • Strong customer service skills and ability to work with diverse populations
  • Graduate with a Bachelor’s, Master’s, or Doctorate in physical therapy from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.
  • Currently licensed in the state of Massachusetts
  • Knowledgeable and experience with Medicare OASIS documentation Preferred
