**Join Our Dynamic Team as a Physical Therapist!** **Job Description:** - Actively manage and optimize patient visit schedules for maximum productivity and patient care. - Conduct comprehensive assessments, including psychosocial and safety evaluations, to tailor individualized care plans. - Develop and document personalized plans that promote optimal functional independence and safety. - Provide skilled physical therapy treatments, ensuring quality care and patient education on every visit. - Collaborate with a multidisciplinary team to coordinate care and communicate effectively about patient needs. - Supervise and mentor Physical Therapist Assistants (PTAs), students, and aides to enhance team efficiency. - Ensure timely and accurate completion of necessary paperwork in compliance with regulations and standards. - Monitor billing processes to guarantee accurate and effective insurance management. - Engage in training programs for healthcare staff and community groups, sharing your expertise. - Stay current in the field of physical therapy and contribute to the promotion of rehab services. - Perform essential patient health assessments and procedures as needed. - Communicate changes in patient schedules promptly to maintain flow and efficiency. - Complete OASIS time points accurately, adhering to agency policies and regulations. - Collaborate with Quality Improvement staff for precise clinical documentation. - Follow agency protocols for McKesson processes to enhance care continuity. - Fill out necessary forms accurately and efficiently during patient visits. - Conduct visits in a time-effective manner while ensuring optimal care delivery. - Assess patient needs for Home Care Aide (HCA) services and coordinate effective care plans. - Ensure safe patient discharges and transfers while processing all related paperwork. - Report patient incidents promptly to maintain high standards of care. - Participate in peer review audits, providing and receiving constructive feedback professionally. **Shift Information:** - Monday to Friday from 8 AM to 4 PM with occasional weekend days. **Minimum Years of Experience:** - 2 years of relevant experience required. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
