Physical Therapist (PT) – PRN

$60–$65/hour | PRN
Bring a practical approach to resident recovery at Vantage Health in Wakefield, MA. Stern Therapy Consultants is looking for a PRN Physical Therapist to evaluate and treat adults in a skilled nursing rehabilitation setting, helping them build the mobility needed for daily activities and their next stage of care.
What You'll Do

Perform evaluations that consider residents' movement abilities, medical precautions, prior function, and personal goals.

Establish individualized plans of care with measurable targets for mobility and physical performance.

Deliver gait training, therapeutic exercise, balance activities, and transfer training.

Determine appropriate mobility equipment and instruct residents and caregivers in its safe use.

Reevaluate progress and provide clinical direction to PTAs involved in treatment.

Complete accurate documentation and work with nursing and fellow therapists on care coordination and discharge planning.

Qualifications

Active Massachusetts Physical Therapist license.

Ability to adapt care to residents with varied diagnoses and functional abilities.

Skilled nursing or post-acute experience preferred; new graduates are welcome to apply.
