About the Role:
Solara Hospice LLC is seeking a compassionate and dedicated Hospice Social Worker to join our care team in Waltham, MA. In this meaningful role, you'll provide essential emotional, psychosocial, and practical support to patients and their families during one of life's most profound journeys. Responsibilities:
- Conduct comprehensive psychosocial assessments for hospice patients and their families
- Develop and implement individualized care plans in collaboration with the interdisciplinary hospice team
- Provide counseling and emotional support to patients and families regarding grief, bereavement, and end-of-life planning
- Connect patients and families with community resources, financial assistance, and support services
- Facilitate advance care planning discussions, including assistance with advance directives and healthcare proxies
- Participate in regular interdisciplinary team meetings to coordinate holistic patient care
- Provide bereavement follow-up support to families after the passing of a loved one
- Maintain accurate and timely clinical documentation in compliance with hospice regulations
Requirements:
- Master's degree in Social Work (MSW) from an accredited program
- Current LCSW or LICSW licensure in the state of Massachusetts
- Prior experience in hospice, palliative care, or end-of-life care strongly preferred
- Knowledge of Medicare/Medicaid hospice regulations and compliance standards
- Strong communication, counseling, and crisis intervention skills
- Ability to work independently in a field-based, patient home environment
- Valid driver's license and reliable transportation required
- Empathetic, culturally sensitive, and team-oriented demeanor
