Job Description
GSK Healthcare & Services is seeking a compassionate and dedicated Home Health Registered Nurse (RN) for a 20-week contract opportunity in Vermont.

The organization provides high-quality home health, hospice, and other community-based healthcare services, ensuring compassionate and accessible care for patients from all backgrounds.
As a Home Health RN, you will provide individualized, goal-oriented nursing care in accordance with physician orders and established nursing standards. You may also serve as a Case Manager when designated as the lead discipline, coordinating patient care and supporting positive health outcomes.
Responsibilities

Provide individualized nursing care to patients in their homes.

Assess patients' health conditions and develop appropriate nursing care plans.

Implement and evaluate care plans in accordance with physician orders.

Monitor patient progress and communicate changes in condition to the healthcare team.

Coordinate care and serve as Case Manager when assigned as the lead discipline.

Maintain accurate and timely patient documentation.

Educate patients and families on health management, treatment plans, and disease prevention.

Collaborate with physicians and other healthcare professionals to ensure continuity of care.
Requirements

Active Registered Nurse (RN) license in Vermont or applicable compact license.

Minimum of 1 year of nursing experience.

Current BLS/BCLS certification.

Home Health or Hospice nursing experience is preferred.

Strong patient assessment, communication, and care coordination skills.

Ability to work independently and provide compassionate, patient-centered care.
