This position is primarily remote, allowing you the flexibility to design your work environment while still being part of a dynamic and supportive outpatient office team. You will be able to manage your schedule effectively, working Monday through Friday with no evenings or weekends required. Key Responsibilities\n

- Engage with clients through one-on-one sessions to explore their goals, preferences, and available resources, establishing a strong therapeutic relationship.
- Create personalized rehabilitation plans for clients that reflect their unique educational backgrounds, abilities, employment aspirations, and overall mental health needs, adjusting these plans as their journey progresses.
- Collaborate closely with a multidisciplinary team, including physicians, occupational therapists, psychologists, and other mental health professionals, to design and implement individualized rehabilitation strategies that are client-centered.
- Document and maintain thorough case files that capture important client information, service utilization, provisions made, and communication logs to ensure seamless care continuity.
- Monitor client progress meticulously, assessing whether goals are being met, and recalibrating rehabilitation strategies as necessary to foster optimal outcomes.
- Communicate consistently with clients throughout their engagement with job training and occupational placement programs, addressing any challenges that arise to ensure a smooth transition.
- Involve clients’ families and support systems in the care process to enhance understanding and improvement of client outcomes.
- Conduct needs analyses and timely reporting to assist in refining rehabilitation services and aligning them with client objectives.
- Make referrals and recommendations to appropriate resources to facilitate comprehensive client care.
