Job DescriptionLPN/LVN – SKILLED NURSINGPosition Overview:

• Provide comprehensive nursing care to residents in a skilled nursing setting.
• Monitor residents' physical and mental status and report changes appropriately.
• Administer medications and treatments according to physician orders.
• Supervise and support nursing assistants/LNAs.
• Manage assigned unit during the shift.

Responsibilities:

• Receive, transcribe, and carry out physician orders.
• Administer medications and treatments and accurately document in electronic MAR/TAR systems.
• Order and maintain unit pharmaceuticals.
• Supervise LNAs and assign resident care according to scope of practice and abilities.
• Perform wound care, including wound VAC care.
• Administer oxygen and gastrostomy/jejunostomy feedings.
• Participate in physician rounds and communicate resident needs.
• Complete admissions, discharges, and transfers.
• Monitor infection control practices and maintain a safe unit environment.
• Review appointments, therapy schedules, vital signs, weights, and nutritional documentation.
• Complete shift reports and review new orders, antibiotics, and precautions.
• Communicate with residents, families, physicians, and nursing leadership.
• Follow all state and federal regulations regarding controlled medications.
• Assist with orientation, staff education, and training.
• Participate in staff evaluations and recommend corrective action when necessary.
• Attend required staff and educational meetings.

Requirements:

• 3+ years of current experience, with experience within the last 6 months – Required.
• Must be willing to float within areas of experience – Required.
• IV certification required; must be willing to obtain during onboarding if not currently certified.
• BLS (AHA) – Required.
• Active New Hampshire license in hand at submission – Required.
• Compact license accepted.
• Previous travel experience – Highly Preferred.
• Rural/small facility experience – Highly Preferred.
