Role: Nurse Practitioner (NP) or Physician Assistant (PA) – Post-Acute Care
Location: Worcester, MA (In-person, rounding across skilled nursing facilities)
Schedule: Full-time, 5 days/week (Dedicated night call coverage provided)
Compensation: Fee-for-service, estimated $160,000–$192,000 annually with uncapped earning potential and ACO shared savings eligibility.
Overview
Take ownership of your practice in a collaborative post-acute setting. You will round on patients, direct individualized treatment plans, and partner closely with facility interdisciplinary teams to deliver high-touch clinical care.Whether you are an experienced clinician or a motivated new graduate, you will step into the role with structured backing: a 3-month onboarding pathway, a 1-on-1 field mentor, proprietary population health data tools, and leadership development programs.
What You Will Do
- Conduct comprehensive clinical evaluations and physical examinations.
- Formulate and adjust personalized care plans to enhance patient recovery and quality of life.
- Partner with facility nurses, physicians, and specialists to coordinate care.
- Counsel patients and families on disease management and treatment goals.
- Maintain prompt, compliant documentation in the electronic health record.
What You Bring
- Active Massachusetts NP or PA license and DEA registration.
- Background in acute, post-acute, clinic, or emergency settings preferred; new graduates are welcome to apply.
- Strong clinical assessment skills, regulatory awareness, and clear communication.
