Location: South Dennis, MA 02660
Duration: 13 Weeks
Schedule: 40 Hours/Week, Monday–Friday
Weekend/Holiday: Occasional weekends and holidays

Position Overview

We are seeking an experienced Registered Nurse (RN) – Home Health for a 13-week contract. The RN will provide skilled nursing care to patients in their homes, develop and manage individualized plans of care, coordinate with physicians and other healthcare professionals, and ensure appropriate documentation throughout the patient's episode of care.

Key Responsibilities

  • Evaluate the clinical needs and conditions of patients receiving home health services.
  • Initiate, develop, implement, and revise individualized patient plans of care as needed.
  • Address identified patient-care needs and document interventions and outcomes appropriately.
  • Participate in interdisciplinary patient case conferences.
  • Evaluate the effectiveness of established plans of care and make necessary recommendations.
  • Provide instruction and documented supervision of the Home Health Aide plan of care every 14 days.
  • Notify physicians and other involved healthcare providers of significant changes in patient condition.
  • Obtain and clarify valid physician orders for all care provided.
  • Complete admissions and required clinical paperwork accurately and within established timelines.
  • Maintain clear, concise narrative documentation reflecting skilled nursing care.
  • Document patient and caregiver education, direct care provided, and patient/caregiver response.
  • Coordinate patient discharges appropriately.
  • Inform patients and physicians of discharge plans and disposition.
  • Complete all required discharge documentation.
  • Refer patients to appropriate community resources when needed.
  • Collaborate with patients, families, caregivers, physicians, and interdisciplinary healthcare teams.
  • Follow applicable home health, patient-safety, and documentation standards.

Requirements

  • Current, active Massachusetts RN License.
  • Current CPR certification required.
  • Previous Home Health nursing experience preferred.
  • Strong assessment, care-planning, documentation, and patient-education skills.
  • Ability to work independently in a home-based care environment.
  • Strong communication and organizational skills.
  • Ability to travel throughout the assigned service area.
  • Availability for occasional weekends and holidays.
