**Join Our Dynamic Team as a Home Care Registered Nurse!** **Job Description:** - Bring your expertise in OASIS to provide exceptional care for our patients. - Assess patients and care givers' needs with the nursing process to deliver top-quality care. - Develop and communicate realistic care plans that involve patients and their families. - Manage assigned cases as a Case Manager while adhering to agency productivity standards. - Master OASIS documentation, staying updated on changes to meet agency and regulatory requirements. - Implement medical plans and nursing interventions to ensure high-quality patient care. - Stay proficient in High-Tech skills, including IV maintenance and complex wound care, while adhering to infection control standards. - Collaborate with Licensed Practical Nurses to delegate appropriate tasks and ensure comprehensive care. - Evaluate care plans regularly to address needs effectively and maintain quality of services. - Keep accurate and up-to-date records documenting nursing care for seamless healthcare team integration. - Follow McKesson protocols for visit management and documentation accuracy. - Educate patients and caregivers on treatment options, medication side effects, and discharge instructions. - Identify inter-disciplinary patient needs and communicate them timely to the healthcare team. - Complete essential Agency, State, and Federal forms accurately during visits. - Conduct visits efficiently while keeping communication open with schedulers for optimal visit management. - Assess patient needs for Home Care Aide services and coordinate plans of care effectively. - Transmit patient incident reports swiftly to ensure quality improvement. - Ensure coordinated and safe discharges or transfers with all necessary paperwork. - Work closely with physicians and team members to deliver safe and effective patient care. - Participate in monthly peer review audits, providing and accepting constructive feedback. - Schedule and facilitate case conferences, enhancing communication amongst team members. - Advocate for patient care issues, tailoring care to meet individual and family needs. **Shift Information:** - Tuesday to Saturday, 8am to 4pm with some on-call requirements, or Monday to Friday with some on-call and weekend shifts. **Minimum Years of Experience:** - 2 years **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
