OCES aims to support elders and individuals with disabilities by providing vital information and coordination of services that promote healthy, save lives for our consumers. We want to help them stay as active and engaged in their lives and in their own communities for as long as possible.
To that end, we are looking for a dedicated, caring professional who enjoys working in the community to work with our Housing Team to ensure services and support to residents on-site in our Brockton area locations.
What you will do:
- Review the needs of residents in order to link to community programs and services.
- Serve as the Multi-disciplinary Assessment Team Leader and meet with the team to assure service coordination.
- Facilitate care conferences with residents, family, and other participants.
- Plan and coordinate social, educational, and recreational programs for residents.
- Prepare and distribute a newsletter to keep residents informed of planned activities.
What you'll bring:
- Your passion for helping others and desire to make a difference
- Adaptability to be able to change as the needs of consumers and/or programs change
- At least two years of experience in the human services field preferably with the elder population
- Bachelor's degree in Social Work or related field preferred
