Job Title: Home Health/Hospice RN
Facility Type/Setting: Home Health & Hospice – Outpatient Care
Shift & Schedule: Fri, Sat, Sun, and Mon; 8:00 AM–6:00 PM
Contract Length: 24 weeks
Guaranteed Hours: 40 hours/week
Call / Weekend / Holiday Requirements: Some call may be scheduled, but only on an emergency basis; Friday–Monday schedule includes weekends
Experience Required / Preferred: OASIS experience required; Home Health and Hospice experience required; experience traveling from patient to patient for home visits

Requirements:

- OASIS (Outcome and Assessment Information Set) experience
- Home Health and Hospice experience
- Experience conducting patient-to-patient home visits
- Ability to manage and adjust to schedule changes
- Valid driver's license in good standing
- BLS required
