JOB DESCRIPTION
As Social Worker for Caring Hospice, you will provide quality hospice care, end-of-life, educational and grief support services. We are currently seeking a Social Worker to conduct hospice assessments and patient visits.

JOB REQUIREMENTS
<ul>
<li>
Must be a Licensed Social Worker (LMSW) or a Licensed Clinical Social Worker (LCSW) in the state of MA.
</li>
<li>
Minimum of 2 years of hospice experience preferred in hospice or oncology.
</li>
<li>
Candidate must have previous experience in conducting comprehensive assessments.
</li>
<li>
Ability to work effectively on an interdisciplinary team.
</li>
<li>
Prior experience making funeral arrangements; patient care planning, accessing community resources and discharge planning.
</li>
<li>
Ability to provide grief counseling/support.
</li>
<li>
Ability to foster open communication with patients, patient’s families, facility partners, staff and other healthcare providers
</li>
<li>
Must have reliable transportation, and be able to travel to where the patient resides.
</li>
</ul>
