🏡 Travel Home Health RN – 13 Week Contract
\n We’re seeking an experienced Home Health Registered Nurse (RN) for a rewarding travel opportunity in Rutland, Vermont!
\n 📍 Location: Rutland, VT
\n ⏳ Contract: 13 Weeks
\n ⏰ Guaranteed Hours: 37 hrs/week
\n 🕒 Schedule: Tuesday–Saturday, 8 AM–4 PM with some on-call OR Monday–Friday with some on-call/weekends
\n Requirements:
\n ✅ Active RN License — Vermont license
\n ✅ Minimum 3+ years of RN Home health experience
\n ✅ OASIS experience required
\n ✅ Strong Home Health experience
\n ✅ Ability to manage patient caseloads independently
\n ✅ Excellent documentation and communication skills
\n Responsibilities:
\n 🔹 Conduct comprehensive home health assessments and develop individualized plans of care
\n 🔹 Complete accurate and timely OASIS documentation
\n 🔹 Serve as Case Manager for assigned patients
\n 🔹 Provide skilled nursing care, including IV therapy, wound VAC and complex wound care
\n 🔹 Educate patients and caregivers on medications, treatments and care plans
\n 🔹 Coordinate care with physicians and interdisciplinary teams
\n 🔹 Evaluate patient progress and adjust care plans as needed
\n 🔹 Coordinate safe patient discharges and transfers
\n 🔹 Maintain accurate clinical documentation and follow agency protocols
\n Ideal Candidate:
\n ✔ Experienced Home Health RN with strong OASIS skills
\n ✔ Excellent clinical judgment and time-management abilities
\n ✔ Comfortable working independently in a home-care setting
\n ✔ Patient-focused, organized and collaborative
