Responsibilities:

• Provide direct patient care under the supervision of a registered nurse, including administering medications, performing vital signs, and assisting with activities of daily living

• Monitor and report changes in patients' conditions to registered nurses and other healthcare team members

• Develop and implement individualized care plans to meet the unique needs of each patient

• Collaborate with healthcare team members to ensure effective communication and coordination of patient care

• Maintain accurate and detailed records of patient care and treatment

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:

• Current LPN license in the state of Massachusetts

• High school diploma or equivalent required

• Previous experience in a long-term care setting preferred but not required
