Job Details:
\n
• Cover patients across Rutland and Bennington County
\n• Actively monitor and amend patient visit schedules to maximize productivity and patient need
\n• Perform comprehensive OASIS and PT assessments including psychosocial, educational, safety, and risk factor parameters
\n• Develop individualized plans of care to promote optimal functional independence and safety
\n• Create patient-specific goals and ensure plan of care reflects logical interventions and progress toward those goals
\n• Provide skilled physical therapy treatment including therapeutic techniques and patient/caregiver education
\n• Assess patient status and progress on each visit to ensure quality of care
\n• Coordinate patient care with physicians, nurses, caregivers, other rehab services, and students as Care Manager and/or team member
\n• Supervise and direct clinical activities of PTAs, PT students, and home care aides
\n• Complete and coordinate required documentation in accordance with federal and state regulations and APTA guidelines
\n• Ensure accurate charging and billing processes and address inconsistencies in insurance coverage
\n• Participate in in-service training programs for rehabilitation, medical, nursing staff, volunteers, and community groups
\n• Carry out patient-related health assessments including PT/INR and blood pressure monitoring
\n• Perform patient-related procedures including staple/suture removal and wound care
\n• Complete OASIS time points in accordance with agency, state, and federal guidelines
\n• Collaborate with quality improvement staff to maximize accuracy of clinical notes and OASIS documents
\n• Follow agency protocol with McKesson processes including locking down visits, follow-up notes, and visit completion
\n• Accurately complete agency, state, and federal forms including HHABN and Medicare Notice of Non-Coverage
\n• Carry out visits in a time-effective manner and communicate schedule changes to scheduler and supervisor in a timely manner
\n• Assess patients' needs for HCA services and coordinate plan of care with patient and caregiver as necessary
\n• Coordinate safe patient discharges and transfers and ensure all appropriate paperwork is processed
\n• Transmit patient incident reports to QI Specialist in a timely manner as necessary
\n• Participate in regular peer review audits and provide and accept constructive feedback professionally
