American Traveler is seeking an experienced RN for a Float Home Health position in Massachusetts, requiring at least 1 year of certified home health and case management experience with an active MA RN license.

- Home health setting serving patients across the North Region, including areas in Cape Ann, Essex, Gloucester, Manchester, Rockport, Ipswich, and surrounding communities
- Float role with travel to patient cases as assigned throughout the region
- Day shift schedule: 8:00 AM – 4:30 PM
- Weekend rotation required approximately once every 6 weeks
- 13-week contract
- EMR: Epic (charting experience required)

Requirements\n

- Active MA RN license required — no pending or conditional licenses accepted
- All certifications must be from the American Heart Association (AHA)
- Minimum 1 year of experience in a certified home health agency setting required — experience in hospice, private duty care, or standalone case management roles does not fulfill this requirement
- Minimum 1 year of case management experience required
- Valid driver's license and ability to travel to patient cases as assigned
- Flu vaccine required; no religious or medical exemptions accepted
- 2 professional references required for consideration
- Active MA RN license must be provided at time of consideration — no pending licenses accepted

- Responsibilities include assessing patient conditions, developing and implementing care plans, and providing education and support to patients and their families
- Performs admission processes and case management responsibilities in accordance with home care policies and standards of practice
- Utilizes thorough and timely electronic documentation and interdisciplinary communications
- Care is delivered in alignment with organizational philosophy, policies, and community health practice standards
- Timekeeping system: UKG/Kronos
- Local candidates residing within 60 miles of Massachusetts General Hospital in Boston are eligible to apply
- Parking is not validated at Boston locations; public parking is available nearby at the candidate's expense
