TITLE: PERSONAL CARE ATTENDANT (ASSESSMENTS) PROGRAM NURSES - FT or PT (RN or LPN) *SIGN ON BONUS*
GENERAL DESCRIPTION:
The PCA program assists people of all ages in Franklin, Hampshire, Hampden, and Worcester County. For this position, we are able to offer FT or PT (at least 20 hours per week or FT at 35 hours per week). We expect that these positions will be available for a 9-12 month period. We are offering a sign-on bonus of 3k for LPN’s and 5k for RN’s.
The Personal Care Attendant (PCA) Program Nurse is responsible for the performance and completion of clinical assessments of individuals who receive MassHealth Personal Care Attendant Services. Assessments occur in-person in the individuals’ home. When possible, nurses complete assessments in regions near their home-base.
The pay for the position is 32.96 for LPN’s and 36.04 for RN’s.
QUALIFICATIONS/REQUIREMENTS:
Requires: Current Massachusetts RN/LPN License; at least one (1) year of clinical and/or nursing in a community or long term care setting.

Computer proficiency to process all consumer records in electronic database

Ability to work independently and as a member of an interdisciplinary team

Working knowledge of a wide range of disabilities, assistive devices and other modalities for maintaining independence and available community resources
ESSENTIAL FUNCTIONS:
-
Assess PCA Consumers' functional limitations, both face-to-face (in the consumer’s home) and by phone/video, to determine the need for PCA Services. Identify disabling diagnoses and other medical conditions.

Complete MassHealth Application and Evaluation for PCA Services in compliance with MassHealth regulations. Meet deadlines for completion and submission of these assessments.

Work collaboratively with Occupational Therapists on Evaluations as needed.

Work in conjunction with Functional Skills Trainers and the PCA Program Director to ensure consistency and compliance of all Evaluations before submission to MassHealth.

Attend LifePath staff training and meetings.

Other related duties as assigned.
SUPERVISION:
The PCA Program RN/LPN reports to the PCA Program Director.
Physical demands:
Visual, speaking, auditory and mobile capacity necessary:
Capacity to see computer screen, read written material, and drive a car.
Capacity to hear and speak on the telephone.
Capacity to communicate verbally with consumers, caregivers, supervisors and managers.
Capacity for fine manipulation in the frequent use of office equipment such as computers, copy machines, fax machines, telephones, calculators, etc.
Capacity to drive in all weather to visit consumers in their homes
Capacity to navigate uneven terrain in all weather to visit consumers in their homes
Capacity to climb stairs
Ability to sit and/or stand for extended periods of time.
Occasional reaching and grabbing objects with both hands, twisting of hand and wrist, and pushing and pulling of objects.
Occasional bending, squatting, and twisting to perform work functions.
Occasional capacity to lift up to 25 pounds.
Mental demands:
Ability to understand and/or interpret complicated program instructions and laws.
Capacity to maintain confidentiality of all documentation and client communications according to HIPPA standards.

Environmental working conditions:
Work is split between indoor office work and traveling outdoors to meetings, trainings and consumers’ homes.
Must be able to tolerate odors and pollutants including but not limited to smoke, air fresheners, pet dander, personal odors, etc. from a variety of in-home conditions.
Must be able to tolerate by-products of office machine operation.
Must be able to tolerate heat and cold of seasonal changes and indoor temperatures.
