Acclaim Home Health is seeking a dedicated and compassionate Physical Therapist to join our Home Health team in Worcester, MA. This role involves providing high-quality physical therapy services to patients in their homes, helping them recover and improve their physical function in a comfortable and supportive environment.
Responsibilities
- Evaluate patients' physical conditions and develop individualized treatment plans.
- Provide skilled physical therapy services in patients’ homes, focusing on rehabilitation, mobility improvement, and pain management.
- Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care.
- Document patient evaluations, treatments, and progress notes thoroughly and accurately.
- Educate patients and their families on exercises and activities to promote recovery and avoid injury.
Requirements
- Licensed Physical Therapist in the state of Massachusetts.
- Experience in home health or outpatient physical therapy preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Valid driver’s license and reliable transportation to visit patients’ homes.
- Current CPR certification.
