We are seeking RN Hospice Case Manager for a travel / contract position in Bedford, NH. Accepting both local and traveler candidates.
Position Details:

Contract Length: 13 weeks
Pay Rate: Range $2080 to $2120 Weekly Gross pay (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments
Unit: Home Health
Shifts available: Days
Shift timings: 8 Hours; Days, 8a – 5p
Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri
Position Requirements:
- Registered Nurse: 2 years - Required
- Experience with Hospice nursing in a home setting: 1 year within the last 3 years - Required
- Home Care Home Base (HCHB) charting experience - Required
- Must be able to do admissions - Required
- Case Management experience - Highly preferred
- Must have valid Driver’s License and Personal Auto Insurance
- Must be Covid Vaccinated or provide medical or religious exemption
Education, License and Certification Requirements:
- ASN or an equivalent – Required
- BSN or an equivalent – Highly preferred
- RN License for State applying or Compact if accepted - Required
- BLS Certification from AHA or American Red Cross - Required
Job Description:
- Provide and coordinate care for assigned hospice patients in the comfort of their homes.
- Develop and implement nursing care plans, including Start of Care and admissions, as assigned.
- Case management and provide clinical oversight to interdisciplinary teams
- Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
- Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status
- Document and complete charting for all patient visits within 24 hours.
- Provide Bereavement Support to patient families.
- Territories to cover: Peterborough and Milford territories
