Position Overview

Seeking a compassionate Primary Care Nurse Practitioner or Physician Assistant to provide onsite primary care to residents in skilled nursing and long-term care facilities.

Key Responsibilities

- Provide primary care to residents in long-term care settings.
- Conduct physical exams and assess medical histories.
- Diagnose and manage acute and chronic conditions.
- Order and interpret diagnostic tests and laboratory results.
- Develop and adjust individualized treatment plans.
- Prescribe medications and monitor treatment effectiveness.
- Collaborate with facility staff and interdisciplinary teams.
- Educate residents and families regarding treatment and care plans.

Qualifications
\n

- Active and unrestricted PA license in Vermont.
- Minimum 6 months of long-term care or skilled nursing experience.
- Strong clinical, communication, and critical-thinking skills.

- Ability to perform required physical and clinical duties.
