Overview

As a Nurse Discharge Planner, you'll collaborate with various healthcare stakeholders including the Discharge Nurse Manager, Regional Clinical Leaders, and Facility Health Services to facilitate seamless transitions for patients returning to the community. Your role involves ensuring timely and appropriate care provision while communicating outcomes effectively. By coordinating with Re-Entry Services and Clinical Leaders, you'll ensure continuity of care and optimal patient outcomes in the community setting.


Responsibilities

  • Collaborate with DOC Re-entry services for discharge planning, ensuring adherence to policies and procedures for patients with mental health issues or serious medical illnesses.
  • Review requests from external agencies regarding medical and mental health transfers back into DOC institutions, determining appropriateness of transfer and coordinating necessary services.
  • Maintain relationships with community agencies and communicate with Medicaid Coordinator to ensure resumption of benefits for released patients.
  • Advocate for medical necessity and continuation of prescribed medications for patients admitted to community facilities, while attending relevant meetings and participating in staff training.
  • Consult with supervisory personnel and appropriate leadership regarding complex cases, identify areas of concern, and facilitate resolution while maintaining security clearance and applying critical thinking skills to varied situations.


Qualifications

Education

  • Graduation from an accredited School of Nursing

Experience

  • Minimum 1 year of experience of care delivery in a managed care environment.
  • Computer data entry experience in MS Word, Excel, and PowerPoint.
  • Previous experience working with care management (CM).
  • Chronic care experience highly preferred.
  • Understanding of care delivery in a correctional environment preferred.

Licenses/Certifications

  • A current nursing license in the state of employment is required.
  • Must be able to attain and maintain security clearance to the facility.
